Accommodations are generally active for the semester. To continue receiving accommodations, you must complete the Accommodation Questionnaire. If you did not complete the questionnaire or you are not currently receiving accommodations complete the Renew/Update Accommodation Questionnaire.

Step 1: Renew/Update Accommodations
If you are interested in requesting or changing your accommodations, please reach out to Emily Decker at emily.decker@keystone.edu for assistance.
Step 2: Documentation
Review the guidelines and submit all required documentation.
Step 3: Make an Appointment
Make an appointment with a member of the Student Success Center to review the Self-Report Form and documentation and to discuss possible accommodations.
Step 4: Receive Accommodation Information
Not all accommodation requests will be approved. You will receive notice regarding your accommodations via your Keystone email.
