Disability Documentation Guidelines

Students requesting accommodations must submit documentation from qualified evaluators and or medical/mental professionals who have direct knowledge of the student’s disability history (a relationship of two years or longer is preferred).

Documentation from External Sources

Relevant information from external sources can help to substantiate the presence of a disability and the need for accommodations. This information should be current and provide evidence of the functional limitations of the disabilities and their impact on the student’s life. Criteria for the source, scope, and documentation content differ by disability type.

Documentation may include:

Academic Accommodations

  • Assessments, reports, and or letters (on official letterhead) from qualified healthcare providers, psychologists, or diagnosticians
  • Information from a previously attended college (accommodation plan) or high school (e.g., 504 plan, IEP, etc.)

Emotional Support Animal/Housing Accommodations 

  • Assessments, reports, and or letters (on official letterhead) from qualified healthcare providers, psychologists, or diagnosticians
  • A completed Request for Housing Accommodation form
  • Most recent vet record and vaccination history (ESA request only) – We do not accept certification for an ESA from online ESA-specific services.

Elements to include in documentation

  1. Diagnostic statement with any related diagnostic methodology -Examples: diagnostic criteria, procedures, assessment instruments, and or test scores
  2. Functional limitations or symptoms to help identify which accommodations are appropriate
  3. The impact the requested accommodation will have in minimizing or alleviating the limitations
  4. Any other additional information regarding the condition
  5. Current or past accommodations

 Any documentation related to a disability should be sent directly to Student Success via email or mail from the student or medical health care provider. Documentation can also be dropped off at the Office of Retention and Student Success.

Please do not submit medical documentation or information to the Admissions Office or your admissions counselor.
