Award Requirements


Some financial aid awards have special requirements which must be completed to ensure funding. The Office of Financial Assistance and Planning must be in receipt of outstanding requirements in order to finalize your award(s).

Outstanding Requirements will be found on your Financial Aid Award Notification and must be complete promptly. For an update listing please log into MyKC or contact the Office of Financial Assistance and Planning at 1-800-4COLLEGE Option 2 or financialaid@keystone.edu

Academic Transcript An official account of your academic record(s). Available at the Office of the Registrar from the institution for which you attend and/or attended.
Birth Certificate/SSN A copy of your federally issued birth certificate and/or social security card. You may obtain copies at your local social security office and/ or Department of Vital Statistics.
Citizenship Verification Naturalization Paperwork, Current Alien Registration Card and/ or United States Passport
College Transcripts An official copy of a student’s academic transcript(s) from all previously attended post-secondary institution(s) of higher learning.
Dependent Special Condition Form The College’s Dependent Student Special Condition Form and all supporting documentation as defined by the changes in circumstances stated. Available at Tuition & Aid / Documents and Forms.
Dependent Verification Worksheet The College’s Dependent Student Verification Form and all supporting documentation as defined within the form. Available at Tuition & Aid / Documents and Forms.
Direct Loan Entrance Loan Counseling If you are interested in applying for a Federal Direct loan it is a requirement to complete entrance loan counseling before the loan can be added to your account. Log into www.studentloans.gov , click on counseling, entrance counseling.
Direct Loan Master Promissory Note (MPN) If you are interested in applying for a Federal Direct loan it is a requirement to complete a Master Promissory Note (MPN) before the loan can be added to your account. Log into www.studentloans.gov, click Complete Master Promissory Note, Subsidized/ Unsubsidized.
Documentation of Child Support Paid Documentation of all dollars paid in the form of court ordered child support. The year for which this documentation is needed will match that of the taxes used for completion of your FAFSA.
Documentation of SNAP Benefits Documentation of all dollars received for SNAP Benefits. The year for which this documentation is needed will match that of the taxes used for completion of your FAFSA.
Driver's License or State ID A copy of your current (not expired) driver’s license or state ID card from you legal state of residence.
FAFSA Signature Page A signature from the student and/or parent to finalize the Free Application for Federal Student Aid (FAFSA). You may e-sign your FAFSA at www.fafsa.gov.
Father's SSN Card/Birth Cert A copy of your father’s federally issued birth certificate and/or social security card. You may obtain copies at your local social security office or Department of Vital Statistics.
Federal Perkins Loan MPN If you are interested in applying for a Federal Perkins loan it is a requirement to complete a Master Promissory Note (MPN) before the loan can be added to your account. Available at Tuition and Aid / Documents and Forms.
Fed Perkins Loan Reference Form The College’s Federal Perkins Loan reference form. Available at Tuition & Aid / Documents and Forms.
Federal Student Aid Report / FAFSA Your Free Application for Federal Student Aid. Complete your FAFSA at www.fafsa.gov.
Financial Aid Award Accept Notification Your notification was mailed to you as part of your financial aid package. Please select the financial aid you wish to use and return the acceptance notification to the College as soon a possible. High School Completion Status Verification of high school completion status. Form available at Tuition and Aid / Documents and Forms.
I-9 Form If you have been assigned a work study position an I-9 must be completed of which 2 forms of identification are required. Acceptable forms of identification and the I-9 form are available at Tuition and Aid / Documents and Forms.
Identity/Statement Educational Purpose This form will need to be signed in front of a notary public or a financial aid official at Keystone College. Form is available at Tuition and Aid / Documents and Forms.
Independent Verification Worksheet The College’s Independent Student Verification Form and all supporting documentation as defined within the form. Available at Tuition & Aid / Documents and Forms.
Marital Status Inconsistency Verification of Student and/or parent marital status (i.e. marriage license, divorce decree).
 
Medical Expense Form If you have an increase in medical expenses and would like our office to review your qualifications for additional funding we suggest you complete the Medical Expense form along with supporting documentation. The form is available at Tuition & Aid / Documents and Forms.

Parent PLUS Loan Application If you are interested in apply for the Federal Direct Plus Loan to cover the student’s balance you may apply at www.studentlaons.gov. Click on Request a Direct Plus Loan and follow the steps. You should get an immediate credit decision.
Parent PLUS Master Promissory Note If the Parent Plus loan has been approved, you must complete a Parent Plus loan Master Promissory Note at www.studentloans.ed.gov. Click on Complete a Master Promissory Note, Parent Plus.
Parent's 2013 Federal Tax Transcript A copy of your parent’s federal tax transcripts. Transcripts may be obtained by using the IRS data retrieval tool located within the online FAFSA application at www.fafsa.gov, by visiting your local IRS office and requesting a print out, by requesting a copy be mailed to you via www.IRS.gov, or by calling 1-800-829-1040.
Parent's Non-Filer Statement If your parent is not required to file a tax return, it is imperative that the parent provide documentation on how they support their household. The form is available at Tuition & Aid / Documents and Forms.
Pheaa Status Notice For Pennsylvania Residents ONLY - additional information to determine eligibility for your Pennsylvania State Grant. Please visit www.pheaa.org and create an account for more information.
Selective Service Registration Registration with the US Selective Service. You may indicate such on your FAFSA or visit www.sss.gov.
Social Security Card A copy of your social security card, you may visit your local social security office to obtain a duplicate copy.
State Grant Enrollment Change For Pennsylvania residents ONLY - additional information to determine eligibility for your Pennsylvania State Grant. Please visit www.pheaa.org and create an account for more information.
Student's 2013 Federal Tax Transcript A copy of your federal tax transcripts. Student’s Lifetime Pell Limit Reached No action necessary on behalf of the student. Notification sent via the Student Aid Report (SAR). The SAR informs the student of outcomes related to then completion of his/her FAFSA. The SAR will indicate if the Federal Pell Grant is no longer a funding option due to student reaching/exceeding lifetime allotment.
Student's Non-Filer Statement If you are not required to file a tax return, it is imperative that the student provide documentation on how you support your household. The form is available at Tuition & Aid / Documents and Forms.
Tuition Remission Form (This requirement is for Keystone College employees and their dependents ONLY)
