Student Activities Program Notification

Please return this completed form to the Student Activities Office at least 10 business days prior to the event for approval. 

If your club does not receive approval within three business days from date submitted please contact the Council of Clubs Perfect or the Director of Student Activities.

An EVENT EVALUATION FORM must be submitted within 10 days after the Event is completed.
